All consumers interested in receiving services through Community Care Service, LLC must meet with a Licensed Clinician for a Comprehensive Clinical Assessment (CCA) to assess your current needs and to ensure that you are recommended for the service or services that are most appropriate to meet your mental health or substance abuse needs. Please be prepared to meet for at least ninety (60) minutes for your initial assessment. Please arrive at least 15 minutes prior to scheduled appointment to complete consent forms.

Please bring a Copy of:

• Insurance card

• Driver’s license or state issued ID card

• Any prior assessments or treatment history
